In order to maintain our fees at competitive levels, we ask that you pay for our service at the time of your visit. If you find that this is not possible, our staff will assist you in developing a payment schedule. A patient is responsible for any fees not paid by their insurance company or Medicare.

Insurance
SMOC works with most major insurance companies. For your convenience, we will be happy to submit claims to your insurance carrier and we accept fee assignment of Medicare claims. A patient is responsible for any fees not paid by their insurance company or Medicare. Contact our Insurance Department at 275-0888 for additional insurance questions.
