The physicians and staff at SMOC realize that your time is valuable, and we will make every effort to respect your schedule while providing the highest quality care. For better service, please make an appointment during our office hours Monday through Friday from 8:30 am to 5:00 pm or visit our Walk In After Hours Urgent Care.

New Patient First Visits
If you are a new patient, welcome. Please remember to bring your insurance card with you to your appointment. We ask that you arrive ten minutes early to your appointment to complete necessary paperwork. You may also print the Patient Forms below and complete them before arriving for your appointment.

Pre-Op Surgical Appointments
We would like to remind all surgical patients to bring your completed history and physical forms, and current insurance card to your scheduled pre-op history and physical appointment.

Fees and Insurance

Fees
In order to maintain our fees at competitive levels, we ask that you pay for our service at the time of your visit. If you find that this is not possible, our staff will assist you in developing a payment schedule. A patient is responsible for any fees not paid by their insurance company or Medicare.

Workers Comp
Contact our insurance department at 275-0888 for any Workers’ Compensation claims or questions you may have.

Insurance
SMOC works with most major insurance companies. For your convenience, we will be happy to submit claims to your insurance carrier and we accept fee assignment of Medicare claims. A patient is responsible for any fees not paid by their insurance company or Medicare. Contact our Insurance Department at 275-0888 for additional insurance questions.

Emergencies, Prescription Refills, and Other Medical Questions

Emergencies
When you have an orthopaedic emergency, please call our Office immediately. Our telephone is answered twenty-four hours a day, seven days a week. If your emergency occurs outside of regular office hours, call 336-275-6318. Our operator will immediately relay your name and phone number to a physician on call. The Doctor or their Physician Assistant will then contact you directly and make arrangements for treatment. You can also visit our Urgent Care Center, which is open weekends and evenings. Call 336-235-2663 for more information.

Prescription Refills
If you are calling for a prescription refill, please call your pharmacist, who will contact us for approval. This process allows your physician and pharmacy to review and complete your request more efficiently. Be certain to call your pharmacy before your supply of medication runs out, to allow sufficient time for the approval process. If you need a new prescription, leave a message and your doctor or physician assistant will get back with you. And please note that we do not fill prescriptions on the weekend. Thank you for your understanding.

Medical Questions
We welcome calls concerning your care & treatment. Our professional staff will be happy to assist you whenever possible. If you need to speak with your doctor, please leave a message. Your doctor or their highly experienced Physician Assistant, who allows us to get back with you more quickly, will return your call as soon as possible. Call 336-275-6318.
